Ceranium Medals are used to purchase Shepherd/Ambassador (Asmodian) - Emmisary/Costudian (Elyos) Gears. You can find the selling NPC in Marchutan Priory.
Also, it is one of the needed materials to purify Shepherd/Ambassador (Asmodian) - Emmisary/Costudian (Elyos) Gears. The purification NPC is available at Major Cities (ex: Pandaemonium & Sanctum). They usually located near the remodeling and amfusion area.

About PVP, two new maps have been introduced to cater. It is Kaldor and Levinshor. You can access them by talking to the Teleporter in these new cities Cygnea and Enshar.

You can obtain Hot Balaur Hearts in Dart Poeta. There are specific mobs that drops the said item.

You can always use the 'Search' function in game in order to find out where the most players are besides the capitals. Kaldor or Levinshor, probably Kaldor is more popular but better just check it for yourself when you are on.

Dark Poeta or Beluslan Hoarfrost for example but you can always check this out and make sure you use /where mobname to find them easier.
