    The insignia you get from the "Chest" on stage 4 i bealive when that Grim Reaper npc appers you get 3 chest and you can get insignia out those are bugged ones...

    Here's the pic you can see 2 isnignia sepperated from the rest i get normaly on finishing instance and those 2 dont count.

    p.s no this arent those "old" ones but this ones i got from that chest about 10-20 mins ago.
