So what exactly can you get ingame without having to donate? what can you even spend ceramium medals on gear wise?

 

You can get Baluar Hearts and cook them.
You can buy a set with those medals.

 

You can buy custodian gears with the ceranium medals, after that you can upgrade those custodian gears to Special Elite Overlord gears that if they get enchanted to +20 are better than the arbiter stuff the only thing you need is patience, a lot of it haha.
