Falling Through the Map/Disconnections

So. This game is just about reaching the point of unplayable, at least for me. If it isn't my game disconnecting every five minutes after being completely fine for hours, or me falling through the map during every boss fight..........are there any fixes for these issues. I like playing on this private server, but I've already missed out on boss loot about three times, and I can barely do anything. Plus, ".resuce" teleports me far away, so using that doesn't help me at all.
